TENS therapy should not be painful. Most patients describe the sensation as a mild, pleasant "tingling," "buzzing," or "pins and needles" feeling under the electrodes. Your physiotherapist will start the intensity at a very low level and slowly increase it until you report a strong but comfortable sensation. If you feel any pain or muscle twitching that is uncomfortable, the therapist will immediately reduce the intensity. The goal is to create a sensory "distraction" from your pain, not to create a new, unpleasant sensation.

This varies greatly from person to person and depends on the TENS settings used. When used at a high frequency (Gate Control Theory), the pain relief is often rapid but may only last for as long as the machine is on, or for a short time afterward (e.g., 30 minutes to a few hours). When used at a low frequency (Endorphin Release), the relief may take longer to set in (20-30 minutes) but can last for several hours (4-6 hours) after the TENS treatment is over.
