Unlike passive treatments where the therapist does all the work, MET is an active partnership. The Principle: Post-Isometric Relaxation (PIR)

Here’s how it works: Your therapist gently moves your joint to the very first point of resistance or “barrier.” They then ask you to perform a very gentle, specific, isometric contraction (you push, they resist) for 5-8 seconds. When you are asked to “relax,” your muscle’s stretch reflex is temporarily inhibited (a state called autogenic inhibition), allowing it to lengthen. Your therapist then gently takes the joint to its new, further barrier. This “contract-relax” sequence is repeated 3-5 times, with each cycle “gaining” more pain-free range of motion.
