We use a stethoscope (auscultation) to listen to your lungs and identify which lobes or segments have retained secretions. We also review any X-rays or CT scans, measure your oxygen levels, and assess the strength and effectiveness of your cough.

Positioning & Percussion

Your therapist will place you in a specific, gravity-assisted position, using pillows and wedges to ensure you are completely comfortable and supported. While you are in this position, the therapist will use cupped hands to perform rhythmic percussion on your rib cage over the target area.

Clearance & Re-education

After 5-10 minutes of drainage and percussion in a position, your therapist will guide you through the Active Cycle of Breathing Techniques (ACBT). This involves deep breathing, followed by “huffs” and controlled coughs to clear the mobilized mucus from your airways.

Safety First: When to Be Cautious

While postural drainage is highly effective, it is not appropriate for everyone, and certain positions must be avoided for some patients. Your safety is our absolute top priority, and our therapists rigorously screen for all contraindications.

Contraindications:

We will avoid or modify the treatment for patients with: uncontrolled high blood pressure (hypertension), recent heart attack or unstable cardiac conditions, severe acid reflux (GERD), recent head or neck injury, unstable spine, or recent surgery. Head-down (Trendelenburg) positions are used with extreme caution.

When to Stop:

Stop immediately if you feel severe shortness of breath, dizziness, lightheadedness, a severe headache, or sharp pain. A productive cough is normal and encouraged; distress is not.

Your physiotherapist will place you in one of several different positions, often lying on your side or with your head slightly tilted down, so that the mucus from a specific, congested part of your lung can drain into the larger, central airways. From there, it can be cleared easily with a huff or a cough. It's a key part of our mucus clearance techniques, especially for patients with conditions like bronchiectasis, cystic fibrosis, or pneumonia, and is often combined with percussion (clapping).

Percussion and vibration are hands-on techniques our therapists use during postural drainage to help loosen very sticky mucus. Percussion is a rhythmic "clapping" or "tapping" on the chest wall with a cupped hand. This creates a vibration that travels through the chest and helps "shake" the mucus loose from the airway walls. Vibration is a finer, more gentle shaking pressure applied with flat hands, typically as you exhale, to help "jiggle" the loosened mucus up the airways, making it much easier to clear. A complete postural drainage therapy session can vary depending on how many areas of your lungs are congested. Each specific drainage position is typically held for 5 to 10 minutes. If you need to drain 3-4 different segments, the positioning part of the session might last 20-30 minutes. When combined with percussion, vibration, and breathing exercises (like the Active Cycle of Breathing Techniques), a full chest physiotherapy session at our Bopal clinic often lasts between 30 and 45 minutes.
